This could be due to missing required photos. Every issue, whether it's been fixed or not, requires a photo to be added in a re-inspection to document the current state.
If all required photos have been taken, you may have a poor internet or data connection. While you can record info in the inspection report and take photos offline on your phone, a reliable wifi or data connection is required to upload photos, sync data, and publish the report.
